  Mass Activity Request Form
  
  This form is mandatory for special activities that will take place during mass. For example:
  A speaker from a ministry/group wishes to speak at the end of mass
  A ministry/group wishes to take up a 2nd collection for a special cause
  A ministry/group wishes to be in attendance in an official capacity
  Please submit this request form to the parish office at least 10 days before the requested mass(es) so that the proper arrangements can be made. Failure to do so will result in your request being denied. Please contact Fr. Paul Graney with any questions.
